Team members should use Zendesk when they receive requests from customers to complete questionnaires and surveys or provide documentation or audits relating to Change Healthcare’s information security or compliance procedures and initiatives. The tool can also be used to submit customer attestation and certifications.
You should use Zendesk if your request falls into one of below categories:
- Customer Security Assurance Requests
- e.g. Assessments; Questionnaires; Third Party Reports/Certifications (SOC, HITRUST); InfoSec Audits;
- Enterprise Compliance Customer Requests
- e.g. CMS Attestations; Offshore Attestations; Customer Audits
- Enterprise Policy Requests
- e.g. Questions about specific Enterprise Policies and requests to update an Enterprise Policy, create a new Enterprise Policy, or archive an existing Enterprise Policy.

How do I check the status of a request after it is submitted?
Log in to your ZenDesk account and check the appropriate ticket No. under "My Requests".
